Request for Leave of Absence
Please complete this form if you wish to request leave of absence for your child. Please note a form needs to be completed for each individual child.
Parents do not have the right to take their child out of school for a holiday during term time. Parents must apply to the Headteacher in advance, who will grant only one holiday period absence during their education at West Lodge.
Leave will not be granted for:
- Children whose attendance is below 96%
- Children in Years 2 or 6 before and during the statutory assessment period (February to May)
